Cancellation/No Show
Policy
Your appointment time is reserved especially
for you. If you are unable to keep your appointment, please give us
at least 24 hours notice. There is a late fee charged for all
appointments canceled without 24 hours notice.
Payment
Payment is due at the time of
treatment. We accept cash & personal checks at the
office. If you need to pay by credit card, your payment will
need to be processed before your scheduled treatment using
PayPal. PayPal is a secure site that accepts all types of
credit cards. PayPal keeps your information secure and only
transfers me the money and none of your credit card information.
Client Intake/Health
History Form
All new clients are required to fill
out a health history intake form. This allows the therapist to
adjust the treatment for a medical condition or medication you might
be taking. This information is important for the
therapist.
Please arrive 15
minutes early for your first
appointment.
